How do people working daytime get a haircut or annual physical?

In my current job the best I can do is leave exactly when my shift is up so I don't actually work extra hours for free. In day time job I can try to take a 1hr lunch break during the day to decrease the freebies. "Overtime" is just getting a 2nd job for another hospital and and the primary employers hate that because then they don't get free overtime. But anything more would be noticeable. And any business errand requires more than 1 hour.
