The loose premise is that he helps people confront every day problems by rehearsing every day scenarios with elaborate sets and actors.
In the last episode, he's trying to get this guy comfortable on a date so he hires all these actors to simulate dates so the uncomfortable guy can watch. Some of the actors just end up making out for hours, so Nathan asks if they have partners - two of the girls have boyfriends so he has them come out to set the next day.
So then these girls go back to "rehearsing" these relationships with male actors, which just means they're making out with these other guys for hours for no purpose, and the boyfriends just watch as another guy takes off their girlfriend's top and say "that's not her that's her character and she's doing a great job."
But the whole thing goes to show how shallow and easily exploited these actors are. He hires an intimacy coordiator which is LOLZY - because its apparent that he could just ask these people to actually have sex on camera and like 1/3 of them would do it just because someone told them it was for an HBO show.
