How do people working daytime get a haircut or annual physical?

In day time job I can try to take a 1hr lunch break during the day to decrease the freebies. "Overtime" is just getting a 2nd job for another hospital and and the primary employers hate that because then they don't get free overtime. But anything more would be noticeable. And any business errand requires more than 1 hour. I guess I'll just do occasional 10am-7pm shift.. they certainly will not mind if I do2hrs of evening coverage without extra pay..
