Amanita phalloides, also known as the "death cap", is a highly poisonous mushroom responsible for most serious and fatal mushroom poisoning cases worldwide. It's believed to have originated in Europe and arrived in the United States in the late 20th century on the roots of imported trees. The death cap is a mycorrhizal fungus that often grows near oak tree roots. It has a white or slightly greenish-yellow cap, white or cream gills, and white spores.
