Amanita phalloides, also known as the "death cap", is a highly poisonous mushroom responsible for most serious and fatal mushroom poisoning cases worldwide. It's believed to have originated in Europe and arrived in the United States in the late 20th century on the roots of imported trees. The death cap is a mycorrhizal fungus that often grows near oak tree roots. It has a white or slightly greenish-yellow cap, white or cream gills, and white spores.

Stage 3: Hepatorenal Phase. The final stage of death cap poisoning happens when the liver and kidneys are so damaged that they begin to shut down. This period is very dangerous and may require extreme medical intervention.

Symptoms of the hepatorenal phase typically start three to six days after eating the mushroom and include:

Loss of liver and kidney function

Jaundice

Low blood sugar

Delirium and confusion
