Posted this on a Playstation board but got taken down for being "too political" even though it's literally about a Playstation game.
Nanomachines can practically make your body do anything in the MGS universe, from making you immortal (Vamp) to helping you stay healthy (like a vaccine).
Therefore, nanomachines is the equivalent of vaccinations in MGS 4.
Early in MGS 4, Liquid Ocelot used Sons of Patriots to render all military guys completely dysfunctional with all sorts of uncontrollable rage and ailments.

The only person who was unaffected by Sons of Patriots was Johnny, the one who often has bowel issues, while everyone else's bodies are unable to function.
It was revealed that Johnny Sasaki is the only character who refused to get injected with nanomachines (vaccine), and he later became one of the heroes in the story.
Note: I personally believe in vaccinations.
I am not aware of Kojima ever making his vaxx stance public in the video games media.
It could be that Kojima is anti vaxx, and he has already shown that stance in MGS 4 using the Johnny Sasaki character.
It could be that this is just a video game and we shouldn't read too much into it.
Even if that game is a Hideo Kojima game that you're often suppose to read very much into.
