It means they've already caved from "pay your $10,000 bill now" to "can you pay $300/month?" At this point, you need to tell a story, any story, about why you can't pay $300/month. Make up insane bullshit like "ah, no way I can afford $200/month! My car is having major troubles and I gotta get it repaired to get to work." Go into great detail about the repairs that need to be made. Make up stuff how your BMW is a total money pit and you gotta replace the alternator. Explain what the alternator is and how you think they were invented. Just get into stupid fucking details that are totally irrelevant to medical billing and payments.
Next, tell them your son just became a tranny and you got spend $500/month on hormones and shit and that you're saving up for a dick amputation surgery.
The point is, you just keep blabbing and blabbing on and on about total bullshit stories for 10+ minutes straight. Just tell a fucking story. Eventually, the person on the phone will get so sick and tired of this and just ask, "how much can you afford a month?"
