I am not an expert in this by any means but I think a transvaginal sonographic assessment in an early gestation (first trimester? Less than 20 weeks?) is supposed to be the best for dating a pregnancy and figuring out the estimated delivery date. If you don't get one of those, then physical exam (as you suggested) is probably your best bet
Measuring the head by ultrasound later in pregnancy is moreso done to assess growth problems, you can't do that by putting your hands on the belly. There are a lot of pitfalls for abdominal ultrasound though so I'm sure they integrate a few different things and use gestalt

thats a misread or something. all of our kids measurements were very close to each other -- ie no measurement was abnormal compared to their other measurements.
