Today, I received a lot of information from (of all places) the hospital that had previously treated him. This is not where he experienced the events causing his death.

First, I received a phone call from the assistant to his surgeon (after hours). She told me she heard what happened, that she's extremely sorry for my loss, etc. She also told me the entire staff is shocked. She told me his doctor would be calling me within the next 10-15 minutes, but that she wanted to help me get whatever info I wanted. She literally logged into Epic and went to his charts at the other hospitals, provided timestamps of when procedures occurred, gave me doctors' names and numbers, etc.

Minutes later, I received a call from the surgeon. Her assistant had actually taken great notes and passed them over to the surgeon and we basically went through the list one-by-one.

Yesterday at 13:45, his blood pressure at MGH was 108/78, pulse 63 bpm, O2 98% on room air. He was then transported to Spaulding Hospital in Cambridge.

He arrived at Spaulding at 14:11. He was "chatting away" according to the intake notes. His vitals were normal.

At 15:56, I received a call from my mom--she was bawling.

At 16:01, I received a call from the doctor at Spaulding: Apparently my dad was in cardiac arrest. He noticed a large amount of "coffee ground emesis" occluding his airway. He called 9-1-1 and EMS was able to secure the airway. All the while, CPR was being performed. It was an unwitnessed arrest.

He arrived at Cambridge Alliance Hospital roughly 10 minutes later (it's only 1000 feet away). We were waiting for updates for another three hours. All the while, he had been going in and hour of cardiac arrest. By about 19:30, the doctor called me to explain the grim news. I explained my father's wishes, and then told him that after four rounds of ACLS drugs, an unwitnessed arrest, etc. I would agree that it is now appropriate to discontinue resuscitation efforts.

Maybe 20 minutes later, the doctor called me back to let me know that he had a pulse, but it was weak and thready. I told him that if he keeps that pulse or if it improves, we will resume resuscitation efforts.

At 20:39, he called me to let me know that my dad passed away at 20:37.

Talking to the surgeon today was incredible. She told me that there's a formal review process happening at MGH for this specific incident and that the same thing his happening at Cambridge Health Alliance. She also strongly suggested getting an autopsy and told me that I need to immediately contact the medical examiner. She mentioned that the entire ICU staff is gutted and they brought in grief counselors specifically for this event.

The medical examiner referred me to the Cambridge Police Department and an officer wants to meet with me tomorrow.

"coffee ground emesis" refers to vomit that looks like coffee grounds because it's actually partially digested blood

he was at an "acute rehab center" CALLED Spaulding Hospital Cambridge, but they weren't able to provide hospital services. Apparently, at least. For example, they had to call 9-1-1 to have him intubated by fucking paramedics.

So he had the bypass surgery, he was recovering at a clinic that has no ability to deal with basic EMS? It sounds like he should have stayed at a real hospital for longer.

He didn't have a bypass surgery. He had a reduction of a colocutaneous fistula and a colocolonic fistula. He recovered, got downgrade a level of care from the ICU about a week and a half ago, and then got downgraded to an "acute rehab facility" THAT LITERALLY CALLS ITSELF A HOSPITAL where he died 90 minutes after arriving.

Holy shit. It sounds like he never should have been cleared to leave MGH then. It's odd they would have rushed him out the door if he wasn't well. They have a superb rating.

How the hell did he get a CF? That shit seems gnarly as hell.

it's actually worse: they told us he wasn't allowed to stay there anymore and made it seem like the hospital was kicking him out. I talked to his surgeon about that and she said no such recommendation would ever come from them.

Since when do insurance companies call the shots? Wouldn't MGH override that? Did the VA call them and say yo get this guy out of there? Usually the docs don't sign off unless someone is good to go. I've never heard of anyone dying 90 minutes after leaving a world class hospital. Who told the docs that he was to be shuffled out?

At the end of the day it's a volume business. You can't make any money if you give super care to one person at a time. They should have never let this guy leave MGH if he wasn't fully cleared to go. To be fair on paper it's one of the best hospitals in the world so it would be odd for them to kick a guy out who's on the verge of death.

he left because he was 100% ready to go.

He had a stomach ache that morning according to my mom. He vomited while unattended, aspirated the vomit (the doctor at that facility told me he didn't have an airway until EMS arrived), and drowned.

A month ago, he got life flighted from NH to MGH because he was in sepsis secondary to a really bad infection from where he had peritoneal dialysis. He had emergency, life-saving surgery that I flew in for. He made a miraculous recovery, was talking and eating, and was excited to begin physical therapy and finally be out of the hospital.

The surgery removed a section of his colon and fixed a colocutaneous fistula and a colocolonic fistula. It was extremely complicated and one of the most difficult cases his surgeon had every encountered. She later told me that success cases like his are so few and far between that it really affects the staff when something like this happens.

How did he get perioneal dialysis and why did it lead to sepsis. Seems like he was ridiculously unhealthy and in awful condition. The CFs also seem like something out of a horror movie.
